Your Patient Portal account gives you access to the following features: View your personal health information by accessing your Patient Medical Information tab. Download your personal health information in PDF or CCDA (XML) format. Transmit your personal health information to another provider or HIE. Communicate with your doctor through Secure Messaging. Track activity related to your personal health information to with the Audit Log feature. Request that an Authorized Representative account be added to your portal account if you desire to share your medical information with a family member or another individual. Reset your Patient Portal Password or Email Address using the ‘My Account’ screen. To connect to third party health application, use your activation key found in ‘My Account’ screen.
